Radar, radio and telephone systems failed and Polish airspace was closed for half an hour around midnight on December 16, 2016, according to RMF FM.
It added that air traffic was ordered to bypass Poland and there were delays at the country's airports.
RMF FM said the breakdown took place during a test of emergency power supplies at the Warsaw air traffic control centre.
The broadcaster added that after the system was disconnected from the mains, all backup batteries failed.
"This was the first case in 25 years of the closure of airspace over Poland," Minister of Transport Andrzej Adamczyk was quoted as saying by RMF FM.
The broadcaster reported it was Adamczyk who informed prosecutors about the incident.
