The survey by pollster CBOS found the opposition Civic Platform (PO) party, which headed the previous government, is supported by 17 percent.
The Kukiz'15 grouping is third on 8 percent, and the Nowoczesna (Modern) party on 5 percent.
Other parties would not exceed the 5 percent threshold needed to make it into the Polish parliament, according to the poll.
The survey was conducted between June 7 and 14 on a sample of 1,029 adults.
